1. SAFE, Inc.
    Consumer run drop-in center for mental health patients, consumers, survivors and ex-patients. We often have music, poetry, other activities, coffee, soda, etc.

2. SAFENET
    SAFENET is a statewide, toll-free helpline to assist low-income Oregonians to find health care and public services. SAFENET assists callers in finding appropriate care in their communities with information and referrals to family planning, primary medical care, immunizations, prenatal care, mental health services, WIC nutrition programs, medicaid and AFS access, Oregon Health Plan information, and services for children with special health needs.

3. SCAR/Jasper Mountain
    The agency has a variety of intensive treatment programs for children with serious problems under the age of 12. Programs include Day-Treatment, Therapeutic Foster Care, Crisis Residential, Assessment and Evaluation, short term and long term residential treatment. The agency has three locations: Jasper Mountain Center 37875 Jasper-Lowell Rd., Jasper OR 97438; SAFE Center 89124 Marcola Rd., Springfield, OR 97478, and SCAR Office 1030 G St., Springfield, OR 97477.

4. SIDS Resources of Oregon
    Provides support and information to parents and others affected by SIDS (Sudden Infant Death Syndrome). Publishes a quarterly newsletter. Lending library accessible through any library in the state. Also provides support and information for anyone affected by the death of an infant, as well as information for expecting parents. 5. SMART(Start Making a Reader Today)
    An early intervention literacy program. Community volunteers read aloud to students and listen to students read in selected elementary schools. Two books are provided to students each month to start a home library. Volunteers work with the same twochildren each week for thirty minutes each.

7. Sacred Heart - Ask a Nurse
    A 24-hour source of free and confidential health care information and referral for symptom-related questions, health information, referrals to Sacred Heart services and programs, referrals to community health services, and referrals to physicians.

8. Sacred Heart - Behavioral Health Services - Inpatient Program
    Provides acute care inpatient mental health services including psychiatric and psychological; comprehensive medical work- ups; marital and family assessments; and discharge planning. This is a short-term mental health unit. Selected day hospital and outpatient treatment is available upon physician referral.

9. Sacred Heart - Breastfeeding Specialist
    Counseling services, prenatal classes, telephone consultation. Lactation bedside support for in-hospital (Sacred Heart) patients. Classes for health care providers on lactation education and issues.

10. Sacred Heart - Center for Senior Health
    Services include: Senior Class, HeartLine, Counseling Services for Seniors, and the Collaborative Nursing Practice through Geriatric Services. Monthly fees are charged for HeartLine ($23/month, $27.50 for installation). Counseling services, individual, group and peer counseling, are free through a grant from LCOG and are open to Lane County residents aged 60 and over. Senior Class membership dues are $10/individual, $15/couple, and free to Providence Medicare Extra members. There are varying fees for the Geriatric Services, depending on services performed by the Nurse Practitioner.

11. Sacred Heart - Cesarean Birth Classes
    Provides class on medical and emotional issues related to cesarean delivery.

12. Sacred Heart - Courageous Kids
    Grief support program for children and adolescents who have experienced the death of someone significant. Ten week support group available 3 times a year. Meets weekly during the school year.

13. Sacred Heart - Education Services - Maternal/Child Care
    Provides childbirth classes, tours and education for parents, expectant siblings tours.

14. Sacred Heart - Emergency Services
    A fully staffed and equipped emergency department which provides complete emergency medical services, including; trauma care, worker's compensation treatment and evaluation, routine outpatient evaluation and treatment and referrals. A physician specializing in emergency medicine is on duty 24 hours a day.

15. Sacred Heart - Family Birth Center
    Provides a full spectrum of obstetrical and neonatal services for mothers and newborns. Departments include: Labor/Delivery/Recovery, Mother/Baby Unit and Neonatal Intensive Care. Sacred Heart General Hospital provides services to all patients in need, regardless of their ability to pay.

16. Sacred Heart - Home Health Services
    Services include registered nursing, physical and occupational therapy, speech pathology, medical social work, home health aide, in-home IV and enteral therapy, hospice skilled assessment, and case management services provided in client's home throughout Lane County to all age groups. Services may be reimbursed by Medicare, Medicaid, and many other compensation plans. Financial assistance is available.

17. Sacred Heart - Hospice
    Provides palliative and supportive care for patients with a limited life expectancy. Provides pain control, 24-hour phone nurse availability, volunteer visits, bereavement counseling and the team approach to care (people of different skills working together) to allow patients to be cared for at home by their families with support. Services include instruction to family care givers, community education and information, and volunteer training classes. Serves all of Lane County.

18. Sacred Heart - Medical Social Work Department
    Provides services to hospitalized patients and their families on a cost-free basis. Services include counseling, post-hospital discharge planning, patient advocacy, and referral to community agencies and services.

19. Sacred Heart - Oregon Heart Center
    Provides full-service cardiovascular diagnostic and treatment procedures. Educational programs and materials available.

20. Sacred Heart - Oregon Rehabilitation Center
    Provides comprehensive inpatient and outpatient physical rehabilitation services. Physical and occupational therapy, rehabilitation nursing, psychology, social work, speech and language pathology, and recreational therapy are available for people with disabilities. Programs include stroke therapy, spinal cord injury program, brain injury rehabilitation, driver's evaluation, adaptive aquatics, nursing home consultation, wheelchair evaluation and wheelchair community skills program.

21. Sacred Heart - Special Delivery Program
    A free program for expectant parents. Membership includes: monthly newsletters that follow the mother's pregnancy with information and advice, gifts and discounts from local merchants, preregistration for hospital admission, hospital tour of birthing facilities, information for formulating a birth plan and opportunity for enrollment in prepared childbirth classes. 22. Sacred Heart - Women's Services/Community Education Services
    Provides a variety of health education programs and services to expectant families, including an early pregnancy course, breast feeding classes, Childbirth courses, expectant siblings tours and adult tours of the family birth center. Many classes emphasize positive life-styles with topics such as fitness, exercise and nutrition. Also available to men as well as women, are the Lifesteps Weight Management Program and support groups.

23. Safe Space Youth Group
    This is a group to offer support, information and opportunities for socializing for kids who are gay, lesbian, bisexual, transgendered or questioning. This is an opportunity for these kids to be themselves.

24. Salvation Army Family Services - Eugene
    The Salvation Army emergency services are designed to assist distressed persons in a variety of emergencies without regard to race, creed, age or sex. Limited material assistance includes: food, clothing, prescriptions, utilities, household supplies and rent assistance. Also providing counseling, spiritual guidance, crisis intervention, substance abuse counseling, financial, individual and family support, and client advocacy. The Salvation Army also administers funds for EPUD's Helping Hand and part of LEC's Member Assistance Program. Walk-ins Monday-Friday 10-12, 1-3; appointments Monday-Friday 8-10, 3-4. Outlying services in Florence and Springfield. We provide new shoes for school age kids 5-16 years old during the beginning of the school year.

25. Salvation Army Family Services - Florence
    Limited emergency assistance: clothing, prescriptions (antibiotics), transportation (gas for doctors appointment or job), rental and utilities assistance, furniture and household items. Also provides client advocacy and individual and family support.

26. Salvation Army Family Services - Springfield
    Limited material emergency assistance: food (residents within our service area with proof of address), clothing, prescriptions (antibiotics only), (limited) transportation, furniture, household items, rental. Also provides counseling, crisis intervention, substance abuse counseling, financial, individual and family support, and client advocacy. Walk-ins Monday-Friday 8-12. 27. Self Help for Hard of Hearing People, Inc.

51. Senior and Disabled Services - Disability Services
    Determines eligibility for and administers federal medicaid and state general assistance programs for disabled adults between 18 and 64. Services include cash grants, food stamps and medical cards for eligible individuals or couples. Also includes social services such as protective services for this population. Additionally provides in-home personal care or attendant care for those who need help to remain in their homes, and specialized living situations such as foster care, residential care, assisted living and nursing home care.

57. Serenity Lane - New Hope Outpatient Program
    An intensive outpatient program designed to meet the treatment needs of the chemically dependent adult. After initial evaluation and assessment, the client enters into an intensive treatment phase and then aftercare. Length of treatment is based on individual needs. Participation in AA/NA will be an integral part of treatment. Men's, women's, and co-ed groups with evening programs available. Medical cards accepted. Services also offered in Florence and Salem.

    Provides employers and the community at large with assessment, evaluation and referral services for people with addictive disease and related disorders. Also family and individual therapy.

59. Service Corps of Retired Executives
    Provides counseling for those entering business for the first time, showing a net loss in business, needing business advice, or seeking financing. Operates under the sponsorship of the Small Business Administration. Counselors are volunteers serving without pay; no fees or charges of any kind.

60. Seventh-day Adventist Church and Health Classes - Eugene
    The Eugene Seventh-day Adventist Church offers the following clinics as a public service: Breathe-Free to Stop Smoking, Nutrition - Vegetarian Cooking Classes. Clinics usually begin in late September and run through May. A Stress Clinic is offered as needed. Printed brochures give dates and times for the various clinics, so anyone interested can call the church office for more information. We will be glad to mail a brochure.

61. Seventh-day Adventist Community Service Center - Springfield
    Used clothing and shoes, and a limited amount of bedding to those in need. Also provides layettes to women who live in Springfield.

62. Sex Addicts Anonymous
    12-step recovery program for people who want to stop their own compulsive sexual behavior. Meeting times are: Thursday 7p, Friday 7p, Saturday 8:30a and Sunday 8p. Call for location.

63. Sexual Assault Support Services
    SASS provides support for survivors of sexual assault, past and present, and harassment, and for friends and family of survivors. These services include: 24 hour crisis line; medical and legal advocacy; community education and prevention; information and referral; immediate counseling services including peer support. Group support for women age 18 and older. Services accessible through crisis line, or stop into office between Monday through Friday 9-5. Appointments are not required for service, but recommended. Groups are free.

64. Sheldon High School - Life Skills Network
    High School and post graduate program for students with disabilities (educationally and vocationally challenging individuals ages 14 through 21). Offers training in functional academic living and vocational skill training.

65. Shriners Hospital
    Offers free care for children with congenital orthopedic deformities, problems resulting from orthopedic injuries, diseases of the bones, joints and muscles, cleft lip and palate, orthopedic complications, scarring, bone tumors, and deformity of the limbs as a result of burns.

66. Siletz Tribal Head Start
    Provides comprehensive developmental services for low-income, pre-school children and social services for their families. Program serves (in this or der): Native American children, children with disabilities, and low-income children.Provides developmentally-appropriate Native American curriculum. Professional staff helps children with language, social/emotional, and physical development. Parent skill-building activities and family health & support services are also available.

67. Siletz Tribe Head Start Program
    Preschool education program that serves children of low-income families with emphasis on serving Native Americans. Provides Native American cultural activities.

68. Siuslaw (City) Public Library
    Provides information, children's services, recreational reading, and other library services to the residents of the district. The district includes Florence, Dunes City, Westlake, and Heceta Head. Has a state employment kiosk for card holders, CD ROM reference for health issues, internet access, and a parenting area with referals and toys. A branch library has been established at Mapleton in the Lions Club building (268-4033) on Riverview Street.

69. Siuslaw Area Women's Center
    Provides counseling, education, and support groups for abused women and their children. Provides access to emergency housing for people in Western Lane County. Obtains restraining orders for domestic violence victims. Outlying services from Walton and west. Provides emergency financial assistance for gas, utilities, rental, mortgage, and prescriptions. Provides transitional housing and case management to local homeless families. Provides Elderhelp services to seniors in Florence/Mapleton area. Operates 24 hour crisis and referral line and a shelter for domestic violence victims and children.

    Provides books, magazines, newspapers, videotapes, audiovisual materials, large print books, and reference service. Offers a collection of adult low reading level materials, English as a Second Language materials, GED materials, and vocationalmaterials. Provides literacy referrals. Offers children's story hour, homebound program, public Internet access, listening stations, meeting rooms, an Employment Department computer kiosk, interlibrary loan, and typewriter and computer for public use.

71. Siuslaw School District 97J - Elementary Counselor
    Elementary school counseling program focusing primarily on educational prevention of social and emotional problems. Individual, small group, parent classes, and family counseling. Weekly classroom instruction.

72. Siuslaw School District 97J - Health Services
    Provides illness evaluation, referrals, health screenings, immunization evaluations, and family services coordination.     The following free clinics are held each month in the Solvang Lobby: Blood Pressure Clinic - every Wednesday at 1:15p; Hearing Aid Clinic - 3rd Wednesday 1p.

74. South Lane Mental Health
    Outpatient counseling, supported housing and medication management offered to adults from South Lane County who request services. United Way helps subsidize medication, evaluation and perscription services. For state paid services, individuals must have a severe mental illness and at least one hospitalization in the last three years. Others are seen on a sliding scale. The waiting list for services is prioritized according to need.

75. South Lane Mental Health - Bohemia Residential Community
    We are a five bed mental health foster home for adults with severe mental illnesses. The service has an orientation to help persons develop the skills needed to transition to living in more independent settings.

76. South Lane Wheels
    Transportation for seniors and people with disabilities and others without transportation in the Cottage Grove, Creswell, and South Lane Districts. Will transport people in this area with 24 hours minimum notice; transport requests with less than 24 hours notice might not be accomodated. Person needing an attendant will need to bring one with them. South Lane Wheels makes one trip to Valley River Center in Eugene each month. There is limited service to Eugene/Springfield for medical appointments for people in wheelchairs. Special contract services for children and families of South Lane Family Nursery and Community Sharing clients. Group trips also available.

77. Southern Willamette Private Industry Council
    Provides job training and stay-in-school services to low income residents and dislocated workers throughout Lane County. Vocational life planning, basic skills and occupational skill training, job placement services for adults. Case managmenet and youth competency development for in-school youth; case management, work experience, job search skills, basic skill training and job placement for out-of-school youth. Retraining and readjustment services for dislocated workers. Initial work search, employment preparation, work experience, basic skills and occupational skill training and job placement for welfare clients.

78. Southern Willamette Private Industry Council - Senior Employment Program
    Employment help for seniors 55 years and older who are low-income and meet eligibility requirements. We do on site training, sponsor class work and assist in job search aimed at unsubsidized employment.

79. Special Mobility Services
    SMS is a private contractor employed by Lane Council of Governments to furnish RideSource transportation services in Eugene and Springfield. RideSource is a curb-to-curb dial-a-ride service available to anyone who, because of disability, is unable to use the LTD fixed-route bus system. Application form explains eligibility; forms available by phone or visit. RideSource Escort is a door-to-door service available to certain frail older persons; contact LCOG outreach office for details. RideSource Shopper is a weekly bus serving specific neighborhoods and grocery stores on specific days. Shopper service also available in Veneta, Elmira, Junction City, and Coburg.

    Florence Special Olympics sponsors participation in organized physical education and recreational activities for mentally and physically disabled children. Activities include bowling and track and field.

    Provides tutoring, academic instruction, evaluation, and consultation for people ages 3-adult.

82. Speech & Physical Therapy
    Provides evaluation and treatment of speech, language, and related cognitive development and skills. Provides treatment for problems with central auditory processing.

83. Speech, Language and Hearing
    Provides evaluation and treatment of children and adults with communication delays or disorders. Offers hearing and speech-language evaluations, speech-language therapy for adults and children, afree support group for parents of children using augmentative communication, and a free support group for adults who stutter.

84. Sponsors, Inc.
    Provides transitional services for men and women ex-prisoners to assist in development of self-sufficiency and a noncriminals life-styles. Services include one to one counseling, and assistance in finding food, shelter and employment. Our drop-in-center provides use of a telephone to assist in housing/job search, as well as use as a message phone. We also serve as a mail-drop for clients. In addition, we have an education program on prison and criminal justice issues and will provide speakers when requested. Residential programs for men and women.

85. Sponsors, Inc. - Women's Transition Program
    This program provides transitional housing and case management services for women who are currently on parole and/or probation in Lane County. The average length of residential program participation is three months. Participants are required to be employed full time, attend vocational training/college full time, or a combination of both. Case management services include self-sufficiency and life skills training, anger and stress management, one-on-one counseling, recovery and substance abuse counseling, and parent/child relationship skills training for those women who have children living with them while participating in the program.

    Springfield Public Library meets the educational, informational, and recreational material needs of the community. Materials include books, periodicals, records, audio cassettes, video cassettes and art prints. Also a growing collection of materials in Spanish, including books, magazines, newspapers, audio and video tapes. Services include reference services, children's programs, and use of the following at no charge: public typewriters, computers, conference room and meeting room. Services free to Springfield residents, $36 per year per family for non-residents.

88. Springfield Community Service Center
    Serves the greater Springfield area in emergency assistance with food, rent, and utilities on a limited basis. No interest loans to ADC recipients. Outlying services to the McKenzie River and part of Goshen.

89. Springfield PTA Council Children's Clothing Fund
    Provides vouchers for new shoes, socks, underwear and used clothing (through Goodwill). Referrals must be made through counselors on the Springfield school district staff and approved by the PTA Clothing Fund Chairperson.

90. Springfield Public Library
    Provides books, magazines, newspapers, videotapes, audiovisual materials, large print books, and reference service. Offers an English as a Second Language program.Provides literacy referrals. Offers children's story hour, listening stations, public Internet access, public meeting rooms, interlibrary loan service, and computers and typewriters for public use.

91. Springfield School District #19 - Indian Education Program
    As the public school agency for Springfield Schools, we provide services for a range of students (ages 5-21) with special needs, including special education, compensatory education, English as a Second Language, teen parents, and Native American students. We provide educational and cultural services or activities to meet the needs of Native American students within the Springfield School District.

92. Springfield Utility Board - Project Share
    Provides financial help with utilities for low-income households in the Springfield Utility Board area when funds are available during the winter months. Springfield Community Center administers the program.

93. Springfield/Marcola Family Resource Centers
    Provides resources, activities and referrals to families living in Springfield and Marcola area; clothing exchange closet; parenting classes. Centers loocated at the following schools: Marcola (933-2411), Brattain (744-6380), Centennial (744-6383), Guy Lee (744-6391), Maple (744-6395), Moffit (744-6399), Mt. Vernon (744-6403), Page (744-6403), Riverbend (988-2511), Springfield Middle School (744-6392) and Community Family Center (744-6769).

94. St. Benedict Lodge Catholic Church - Sister John's Program
    Provides: food boxes, clothing gifts at Christmas, caring tree at Christmas, shoe vouchers, limited rental and electric assistance, family visits, emergency gas, emergency trips to town, furniture and appliances (as available), lots of TLC. Food for Lane County distributes food at Lazy Days Trailer Park on alternate Tuesdays.

95. St. Mary's Breakfast
    Free breakfast of potatoes, eggs, meat, oatmeal, milk, coffee and juice. Usually all-you-can-eat.

96. St. Vincent de Paul - Eugene Service Station
    Day access center for homeless and low income adults without children. The station has laundry facilities, showers, brown bag lunches, job networking, information and referrals.

97. St. Vincent de Paul - First Place Family Center
    Day center for homeless families with children or pregnant women. Free showers, laundry, message/mail service, child care, food, clothes, household goods, phone use, advocacy, parenting classes, housing, employment and training information. Also provides intake for Interfaith Emergency Night Shelter program and transitional housing program. Night shelter operates September to mid June, seven days a week; Day shelter operates year round, Monday-Friday 8-5.

98. St. Vincent de Paul - OLPH Conference
    Provides limited assistance with clothing, transportation, utilities, furniture, appliances, and miscellaneous. Help is based on need. SVDP will usually accept and pick up donations of clothing, furniture, and appliances in satisfactory condition within the city limits of Cottage Grove and Creswell.

99. St. Vincent de Paul - Second Chance Renter Rehabilitation Program
    Participants attend 8 workshops designed to increase self esteem, access problem solving techniques, education in rights and responsibilities in a landlord/tenant relationship. Program graduates receive a certificate of completion and a letter of reccomendation. We guarantee their next landlord that we will reimburse the landlord for any expenses caused by the tenant in the first 6 months. We have used that fund twice in the history of our program.

100. St. Vincent de Paul Society
    Provides emergency food boxes, clothing, appliances, furniture, household goods, rent, utility assistance, prescription medication, transportation, and loan of medical equipment. Requests are taken at the office; volunteers notified for home visit to needy families.

101. Study Skills Department
    Offers remedial and developmental classes in the areas of study skills, spelling, grammar, reading, writing and math. A computer lab provides keyboarding and computer skills for individuals with disabilities.Instruction is in small group settings with an instructor who follows a designed curriculum with prescribed course objectives. The clientele is composed of typical college students who share the need to develop their basic skills.

102. Support Coalition
    Local meetings to discuss and plan actions about issues such as alternatives to traditional psychiatry, forcible psychiatric drugging, and coercive electroshock. International: a popular publication called 'Dendron' that connects hundreds of individuals and groups concerned about freedom in psychiatry. A library of free information. Support Coalition Northwest is allied with similar groups called Support Coalition International.

103. Support for Independent Living with Visual Impairment
    A support group which provides physical, emotional and recreational support for people and relatives with vision problems.

104. Sylvan Learning and Technology Center
    Services include: diagnostic testing, prescriptive learning, reading, beginning reading, writing, math, algebra, SAT preparation, study skills, licensing exams,remedial enrichment learning programs, licensing exams for nurses, pilots, insurance, financial services, and GRE tests.
